Our mission is to make patient care as simple as possible.

The staff of Regional Primary Care welcomes you to our office. We hope that all of your interactions with us are both pleasant and productive. Please refer to the information on this page to make your office visit as smooth and efficient as possible.

Appointments

To make, reschedule, or cancel an appointment please call our office at (573) 332-6000 during regular business hours. You may also access our Patient Portal to request an appointment electronically.

Preparing for Your Visit

Please bring your completed packet of office forms, a complete and accurate list of your medications and/or the original bottles, proof of insurance (card), and copayment (due at time of service).

Using the corresponding links within “Helpful Forms”, please complete and submit the necessary forms 24-48 hours prior to your visit. Alternatively, the forms may be printed, filled out, and brought to our office on the day of your visit.

Online Forms

All new patients should complete the New Patient Form AND Consent for Care Form.  Complete the Medical Records Release Consent Form only if you wish us to request your medical records from your previous provider.

Complete the Annual Wellness Form and the Consent for Care Form.

If at any time your personal circumstances change, you may complete the Consent for Care Form to update HIPAA Release of Information and preferences and/or complete the Patient Information Form to update demographic, insurance, and preferred pharmacy information.  If we need to request medical records from or provide your medical records to another healthcare provider, please complete the Medical Records Release Consent Form.  If you wish to complete Advance Directives, please complete the Advance Directives/Power of Attorney Form.

Paying for Your Visit

Payment of any co-pay, deductible and/or any non-covered services will be expected at the time of your visit.  All covered services will be submitted to your insurance carrier.  Click here for a list of insurance companies for which we are providers. Regional Primary Care accepts cash, personal check, MasterCard, VISA, Discover, and American Express.  You may click the convenient Pay A Bill link at the top of this page to pay your bill online.

How to Request Medication Refills

Please call your pharmacy to request a general medication refill.  The pharmacy will notify us if no refills are remaining and we will send approval directly back to your pharmacy.  Refill for controlled or narcotic medications must be requested by calling our office at (573) 332-6000 during regular business hours.  Medication refill requests may also be made using Patient Portal.  We ask that you allow 24-48 hours for your refill request to be completed.

Contacting us after hours

When contacting our office for a medical problem after hours or on the weekend, you may reach the physician on call at
(573) 278-4856. You will be asked to enter a telephone number at which you can be reached and a physician will return your call as soon as possible. If you have a medical emergency,
please dial 9-1-1 or go to the nearest Emergency Room as soon as possible.

Obtaining Test results

Call our office during regular business hours at (573) 332-6000 to leave a message for your physician or their clinical staff to return your call. If the test result has been received by our office, they will communicate the results to you and answer any questions you might have at that time. If we have not yet received the results, we will verify your preferred contact information and return your call as soon as possible.

This tool allows you to access your Personal Health Record, exchange secure electronic messages with our office, request medication refills, view test results, request appointments, and obtain patient education material regarding a variety of health topics.
